Critical Incident Investigation: Fatal Mine Site Rollover

The Situation

A driver at a Western Australian mine site was fatally injured after his truck struck a windrow and rolled while negotiating a turn near a stockpile area. WA Police and the state mines safety regulator opened formal investigations. In parallel, legal counsel acting for the mining contractor engaged us to conduct an independent investigation that could withstand scrutiny in a coronial and regulatory setting.

Our Approach

We mobilised investigators to site while the scene and physical evidence were still intact. The investigation covered scene examination, vehicle examination, human factors, witness management and scene reconstruction. Findings were documented to an evidentiary standard, supported by more than fifty annexures including training records, witness statements, telemetry data and scene mapping.

The Outcome

We ruled out every other explanation and identified the one factor that mattered, giving the client's solicitors a report they could rely on immediately, one that held up through the full regulatory and insurance process with no follow-up queries or gaps to fill. The result was a clean, single-pass resolution, letting the client turn their full attention back to their people and their operation.
