The Brief
We were engaged to investigate a collision between a light vehicle and a loaded haul truck on a haul road at a mine in the Pilbara region of Western Australia. The driver of the light vehicle was fatally injured.
Our Approach
We mobilised two experienced investigators who attended the remote site that same day to commence an investigation. We conducted a full scene and systems review, working alongside site leadership and the state mines safety regulator to reconstruct the sequence of events and identify every contributing factor, not just the immediate cause.
The Outcome
The investigation identified the contributing factors clearly and without ambiguity, providing the client a report they could rely on immediately. Corrective actions have since been introduced, closing the gaps that contributed to the incident and leaving the operation safer for it.
